Registration Requirements
Use the links below to download and print the necessary forms. Save time by completing your forms at home. For each person wishing to attend, each of the following must be turned in to Pack 151's Outings Chair:

• A completed Fall Festival Registration Form
• A completed Annual BSA Health & Medical Record
• A photocopy of participant's medical insurance ID card (front & back)
• A completed Camping Participant Waiver
• Full Payment
