New REQUIRED Annual Health & Medical Record

At the beginning of each Scouting year, each Cub Scout must submit to the pack's Outings Chair an updated Annual Health & Medical Record. Anyone (parents, siblings, etc) going to camp needs one too. We recommend filling one out for everyone in the family who might attend one of our pack meetings, camps or outings.  We'll keep them securely on file.  Better yet, if you turn in all your families' completed forms in the beginning of the Scouting year, we won't need to bother you for them again until the next year.

We'll be using the most recent (2014 printing) streamlined version of the BSA's Annual Health & Medical Record (hereafter called the AHMR) The form is the result of several BSA teams (professionals and volunteers) joining forces to make the form easier to complete and submit.

90% of our Scouts will only need to complete this form. Use it for general scout activities, SHORT term (72 hours or less) camping trips and outings. For example, Fall Festival, Polar Cubs, day camps, summer resident camp, etc. 2014 printing. 


Only a select few scouts need this form.  Use it for LONG term (72 hours or more) camping trips. These will likely be extended stay Webelos-related campouts or high-adventure camps.  Note that Part C requires a physical exam.
